Breathless cough: possible causes and next steps

A cough may irritate the airways while inflammation or narrowing makes breathing feel difficult. Acute bronchitis and a common cold can cause cough and chest discomfort. Asthma can bring wheeze, chest tightness and breathlessness, sometimes after exercise or exposure to a trigger. Allergic rhinitis may feed a cough through nasal drainage. Acid reflux can irritate the throat, particularly after meals or when lying down. Bronchiolitis is a concern in young children. A cough lasting beyond the expected illness period, or one with repeated mucus, deserves clinical review; bronchiectasis and chronic cough are examples of conditions clinicians may consider.

Rest, drink fluids, avoid smoke and notice when symptoms occur. Sit upright if lying down worsens the feeling, and use a prescribed reliever inhaler exactly as directed if you have one. Do not start antibiotics or someone else’s inhaler without assessment. Record wheeze, fever, mucus, chest pain, possible allergens, reflux symptoms and whether breathlessness limits speaking or walking. This information helps a clinician decide whether you need an examination or breathing tests.

Call emergency services for severe breathing difficulty, blue or grey lips, confusion, fainting, inability to speak in full sentences, or sudden swelling of the face or throat. Arrange urgent care for rapidly worsening breathlessness, coughing blood, severe chest pain, or a child who is struggling to breathe. Prompt medical review is sensible when symptoms keep returning, disturb sleep, or follow a new medicine or exposure.
