The brachioradialis helps bend the elbow, especially when the thumb faces upward. Repeated gripping, lifting or pulling can make the outer upper forearm painful, but nearby joints, muscles and nerves can produce similar symptoms.
What the diagnosis means
Despite the familiar word tendonitis, persistent tendon pain is not always explained by inflammation alone. A clinician may use the broader term tendinopathy when the tendon has become sensitive to load. Typical clues include local tenderness and pain reproduced by resisted elbow bending or a firm handshake.
Assessment matters because discomfort near the outer elbow or forearm may instead come from another tendon, the elbow joint, the neck or an irritated nerve. The examination usually considers the exact pain location, recent changes in training or work, strength, sensation and movements that reproduce the problem.

Temporarily reduce the grip force, lifting volume or exercise that repeatedly provokes symptoms, while continuing comfortable daily movement. Avoid repeatedly testing the sore action. A cold or warm pack may be used for comfort with a cloth barrier, and medicine should be checked with a pharmacist or clinician if you have other conditions or take regular medication.
As pain settles, rehabilitation commonly rebuilds tolerance through controlled elbow and wrist exercise, followed by gradual return to the tasks that caused trouble. A physiotherapist can adjust technique and loading when progress stalls. Complete rest for a prolonged period may leave the forearm less prepared for work or sport.
When to seek care
Arrange an assessment if pain keeps returning, interferes with sleep or normal use, or does not improve after sensible load changes. Seek prompt care after a sudden injury with a snap, major swelling, bruising, visible deformity or a clear loss of elbow or hand strength.
Urgent evaluation is also appropriate for a cold or pale hand, rapidly increasing swelling, spreading redness with fever, persistent numbness or difficulty moving the fingers. These features are not routine signs of tendon overload and need a broader medical check.
