Both leg swelling below the knee: what to know

Both-sided swelling often affects the ankles and feet and may be more noticeable after standing or sitting. Varicose veins can impair the upward flow of blood and cause heaviness, aching, or skin changes. Some medicines, reduced movement, excess body weight, and high salt intake can also contribute. Heart failure and chronic kidney disease are medical causes that need assessment; swelling alone cannot identify which explanation applies.

Notice when the swelling began, whether it changes through the day, and whether pressing the skin leaves a temporary dent. Walk gently if you can, avoid sitting still for long periods, and raise your legs when resting. Do not stop prescribed medicines or start water tablets without advice. Arrange a primary-care review if the swelling persists, returns, or comes with breathlessness, reduced urine, or rapid weight change.

Seek urgent help for swelling with sudden breathlessness, chest pain, faintness, coughing blood, or severe weakness. Get prompt medical advice if one leg becomes much more painful, red, hot, or rapidly larger than the other, or if the skin becomes sore or breaks down.

Ask whether your medicines, circulation, heart, kidneys, or liver need checking; whether blood or urine tests are appropriate; and which movement or compression measures are safe for you.
