How the timing can offer clues
Ovulation pain is often felt on one side of the lower abdomen around the middle of a menstrual cycle. Release of an egg and a small amount of fluid can irritate nearby tissue, while bowel gas or fluid retention may add a bloated sensation.
Pain that does not follow a mid-cycle pattern may have another explanation. Endometriosis can present with cyclical pelvic pain, and ovarian cysts can present with pelvic pressure or pain. Neither condition can be identified from bloating alone.
Managing and tracking symptoms
Note the first day of each period, the side and location of pain, how long it lasts, and whether bloating, bleeding, discharge, bowel changes or pain during sex occurs. Repeated mid-cycle timing can help distinguish ovulation discomfort from less predictable pain.
A warm pack, gentle movement and rest may ease mild symptoms. Ask a pharmacist or clinician which pain relief is appropriate if you are pregnant, trying to conceive, taking other medicines or have a health condition.
When pain is not typical
Arrange a medical review if episodes are becoming more painful, last beyond the expected cycle window, repeatedly interrupt normal activity, or occur with pain during sex, unusual discharge, bowel symptoms or bleeding between periods.
Seek urgent care for sudden severe one-sided pain, fainting, shoulder-tip pain, fever, repeated vomiting, heavy bleeding, or pain when pregnancy is possible. These features require prompt assessment regardless of cycle timing.
