Bleeding for no clear reason when it is not your period

The blood may come from the vagina, cervix, womb, urinary tract, or rectum, so identifying where it starts is useful. Hormonal changes, contraception, infection, pregnancy-related problems, growths such as polyps or fibroids, and irritation can all cause unexpected vaginal bleeding.

A small smear after sex is different from bleeding that soaks clothing or returns repeatedly. Note whether the blood is pink, red, or brown and whether you also have pelvic pain, unusual discharge, pain during sex, urinary symptoms, or missed periods. These details help a clinician narrow down the source and decide whether an examination or tests are needed.

Irregular ovulation can make the womb lining shed at an unexpected time. This can occur around the menopause transition and with conditions that affect ovulation, including polycystic ovary syndrome and premature ovarian insufficiency. Bleeding after menopause is not considered a menstrual period and needs medical assessment even if it happens only once.

Record when the bleeding began, how much there is, whether it follows sex, and any associated pain or discharge. Check the name and timing of contraception, hormone medicines, blood-thinning medicines, and recent missed doses. Do not stop prescribed medicine without clinical advice.

Take a home pregnancy test if pregnancy is possible, including when contraception was used. Arrange a routine appointment for unexplained bleeding that continues, comes back, or differs from your usual cycle. A clinician may discuss pregnancy testing, a pelvic examination, infection testing, cervical screening history, blood tests, or imaging according to your symptoms.

Seek urgent medical care for heavy or rapidly increasing bleeding, fainting, severe weakness, breathing difficulty, severe pelvic or abdominal pain, shoulder-tip pain, or bleeding with a positive pregnancy test. These features can signal substantial blood loss or a pregnancy complication.

Contact a clinician promptly for any vaginal bleeding after menopause, bleeding with fever or offensive-smelling discharge, or repeated bleeding after sex. The need for assessment comes from the bleeding pattern and accompanying symptoms, not from assuming a particular diagnosis.

Ask which body site appears to be bleeding, whether pregnancy or infection testing is appropriate, and whether your contraception or medicines could be contributing. You can also ask what change in flow, pain, or general wellbeing should prompt urgent care and what follow-up is needed if the first tests are normal.
