Bleeding During Miscarriage: What to Expect and When to Get Help

The amount and pattern can change as pregnancy tissue passes. Bleeding alone cannot confirm that a miscarriage is complete, so follow the assessment and follow-up plan provided by your maternity or gynaecology team.

Cramping and bleeding may build, become heavier while tissue passes, and then ease. Blood can be bright red or darker, and clots may occur. The experience varies with the stage of pregnancy and whether the miscarriage is happening naturally or after medical treatment.

An ultrasound and, in some situations, blood tests help clinicians check the pregnancy location and whether tissue remains. Bleeding that settles does not by itself show that the process is complete.

Use sanitary pads so you can observe the flow, clots and any tissue; avoid tampons or menstrual cups while bleeding. Note when bleeding began, how often pads need changing, pain severity and any fever. Take only pain relief recommended for you, drink fluids and arrange for a trusted adult to stay nearby if the flow is increasing.

Contact your care team for instructions about follow-up. Depending on the findings and your preferences, options may include waiting, medication or a procedure to remove remaining pregnancy tissue.

Go to an emergency department or call local emergency services if bleeding rapidly soaks pads, you feel faint or collapse, pain is severe or one-sided, you have shoulder-tip pain, difficulty breathing, confusion, fever, chills or foul-smelling discharge. These features can signal major blood loss, infection or a pregnancy outside the uterus.

Promptly contact your maternity team if bleeding or pain is increasing, symptoms return after improving, or you are unsure whether your follow-up is complete. Tell clinicians your blood group if known and bring details of medicines and previous scan results.

Ask whether you need an ultrasound or blood tests, what bleeding pattern to expect with your chosen management, which pain medicines are suitable, whether your blood group changes treatment, and how completion will be confirmed. You can also ask when sex, exercise and attempts to conceive may be appropriate for your circumstances.
