Bleeding and pain during ovulation

Ovulation pain, sometimes called mittelschmerz, is often felt on one side of the lower abdomen and may switch sides between cycles. A small hormonal shift around egg release can accompany light spotting. Cycle timing can suggest ovulation but cannot confirm that it is the cause.

Ovulation-related discomfort tends to occur near the middle of a cycle and settle without steadily worsening. Pain that is strong, lasts beyond the expected window, occurs at other cycle stages, or comes with pain during sex or bowel movements may need investigation for another pelvic condition.

Ovarian cysts can present with one-sided pelvic pain, pressure or sudden sharp pain. Endometriosis may present with cyclical pelvic pain, difficult periods, pain during sex or bowel symptoms. Infection and pregnancy-related problems are other considerations when discharge, fever, a late period or pregnancy possibility is present.

Mark the first day of each period and record when pain and spotting appear, which side hurts, how long symptoms last, and whether they interrupt activity. Also note nausea, bloating, discharge, urinary symptoms, pain during sex and contraception use. This record helps show whether there is a consistent mid-cycle pattern.

Rest and a warm pack may help mild discomfort. Check with a pharmacist or clinician before choosing pain relief if pregnancy is possible or you have stomach, kidney, bleeding or medicine-related concerns. Take a pregnancy test if a period is late or pregnancy could have occurred.

Seek urgent care for sudden severe pelvic pain, pain with vomiting, faintness, marked dizziness, heavy bleeding, fever, or a rigid or very tender abdomen. Urgent assessment is also needed when pregnancy is possible and pain or bleeding occurs, particularly with one-sided pain or shoulder-tip pain.

Book a non-emergency appointment when spotting or pain repeatedly returns, becomes harder to manage, no longer follows a mid-cycle pattern, or is accompanied by pain during sex. An examination or ultrasound may be offered according to the clinical picture.

Ask whether your diary fits ovulation pain, whether pregnancy or infection testing is appropriate, and what findings would make an ultrasound useful. If symptoms recur each cycle, ask about options that may suppress ovulation and whether those options fit your health and pregnancy plans.
