Occasional bleeding after sex: what to know

A small amount of blood may follow irritation of vaginal or cervical tissue, especially with dryness, vigorous penetration, or sex close to a menstrual period. Cervical inflammation or an infection such as chlamydia can also cause contact bleeding. Genitourinary syndrome of menopause can present with dryness, delicate tissue, burning, and bleeding after contact.

Cervical dysplasia, cervical cancer, and vaginal cancer can present with bleeding after sex, but this symptom has many non-cancerous explanations. An examination helps locate the source rather than relying on the colour or amount of blood.

Note when the bleeding occurs, whether it is spotting or a flow, its relationship to your period, and any dryness, discharge, odour, urinary symptoms, or pelvic pain. Pause penetration until bleeding and soreness settle. A water-based lubricant and slower, gentler activity may reduce friction, but recurring bleeding still needs clinical assessment.

Arrange a routine appointment if episodes continue. A clinician may discuss pregnancy possibility and infection exposure, examine the vagina and cervix, offer infection testing, and check whether cervical screening is due.

Seek urgent care for heavy bleeding, faintness, marked weakness, severe or worsening pelvic pain, fever, or bleeding with possible pregnancy. Contact a clinician promptly if bleeding keeps returning, occurs after menopause, appears between periods, or comes with unusual discharge, pain during sex, or a new pelvic symptom.

Ask where the blood appears to be coming from, whether infection testing or a pregnancy test fits your situation, and whether your cervix looks inflamed or easily irritated. You can also ask whether cervical screening is due and what follow-up is needed if the examination is normal but bleeding returns.
