Bleeding After Sex Following a C-Section

Caesarean delivery does not prevent lochia because blood and tissue from the healing uterus still leave through the vagina. Penetration may coincide with that flow or irritate dry tissue. Heavier, worsening, or recurrent bleeding needs clinical review.

The number of weeks since birth changes the context. Early on, lochia and internal uterine healing remain relevant. Later, a menstrual period may return, while breastfeeding-related dryness can continue. Sex that is uncomfortable can create a small vaginal graze.

Bleeding from the cervix may be caused by ectropion, a polyp, inflammation, or infection. Chlamydia can present with postcoital bleeding. Less commonly, cervical dysplasia or cervical cancer may present with this symptom; the bleeding pattern cannot confirm any of these conditions.

If bleeding is light and you otherwise feel well, stop penetration, use a pad, and observe whether it settles. Avoid pressure on a tender abdominal scar. When recovery feels comfortable, try slower activity and water-based lubricant, stopping if blood or pain returns.

Contact your midwife, obstetrician, or GP when bleeding recurs, becomes heavier, or starts after lochia had ended. Share the birth date, bleeding amount, clotting, pain, discharge, breastfeeding status, contraception, and any wound symptoms. A clinician may examine the vagina and cervix or test for infection.

Call emergency services for sudden heavy or gushing bleeding, fainting, severe weakness, a pounding heart, breathing difficulty, or intense abdominal or pelvic pain. A recent caesarean and intercourse do not make these symptoms suitable for home monitoring.

Get prompt medical advice for fever, foul-smelling discharge, worsening lower-abdominal pain, large clots, painful urination, or an incision that is increasingly red, swollen, painful, open, or leaking. These symptoms may require treatment for infection, bleeding, or a wound problem.

Be ready to explain when postpartum bleeding had been fading, how sex changed it, and whether pain is vaginal, pelvic, or near the incision. Ask if healing is sufficient for penetration, whether screening or swabs are indicated, and how to manage dryness without interfering with contraception.
