Bladder cancer that has spread to bone

The term “bone mets” means metastatic disease in bone. It remains bladder cancer rather than becoming primary bone cancer. Treatment is planned around where disease is present, previous therapy, symptoms, organ function, and the person’s goals.

Bladder cancer in bone may present with persistent, localised pain that can worsen with movement or at night. A weakened bone can fracture. Disease in the spine can press on the spinal cord or nerve roots, affecting strength, sensation, and bladder or bowel control.

Bone breakdown can also release calcium into the blood. This may cause unusual thirst, frequent urination, constipation, nausea, drowsiness, or confusion. Imaging and blood tests help the cancer team assess affected areas and related complications.

Tell the oncology team about new pain, reduced mobility, weakness, numbness, falls, or changes in urination or bowel control. Describe the exact location, what triggers the pain, whether it wakes you, and how it affects walking or sleep. Do not wait for a routine visit if symptoms are worsening.

Care can combine treatment for cancer throughout the body with radiotherapy to painful or vulnerable areas, pain medicines, and medicines that reduce skeletal complications. Surgery or stabilisation may be considered when a bone is at risk of breaking. A rehabilitation team can advise on safe movement and mobility aids.

Get emergency help for new leg weakness, difficulty walking, numbness around the buttocks or groin, loss of bladder or bowel control, or severe new back or neck pain. These can indicate spinal cord compression. Sudden intense pain after a minor movement, inability to bear weight, marked confusion, or extreme drowsiness also needs urgent assessment.

Ask: Which bones are affected, and is any area at risk of fracture or spinal cord compression? What is the aim of each proposed treatment? How should breakthrough pain be managed? Are there movements I should avoid? Will I need dental assessment before a bone-strengthening medicine?
