Biochemical miscarriage: what it means and what happens next

It may follow a positive home or blood pregnancy test, then bleeding around or after the expected period. The loss is real even though no pregnancy sac was visible, and emotional responses vary.

Clinicians may repeat a blood test for human chorionic gonadotropin, often called hCG, to see whether the level is declining. A transvaginal ultrasound may be used when dates, symptoms or test results leave uncertainty, but a pregnancy this early can be too small to locate.

Follow-up matters until the result fits a completed early loss or another diagnosis. This is because pain, bleeding and an abnormal hCG pattern can also occur when a pregnancy is developing outside the uterus. A biochemical miscarriage is generally not caused by exercise, sex, travel or something you ate.

Keep any arranged blood-test or scan appointments, even if bleeding has stopped. Use pads if you want to monitor the amount of bleeding, and choose pain relief that your clinician or pharmacist says is suitable for you. Most losses at this stage complete without a procedure, although management depends on symptoms and follow-up findings.

Tell your care team if your blood group, medicines or previous ectopic-pregnancy history may affect the plan. If you want to try for pregnancy again, discuss timing, folic acid and whether any personal health condition needs attention first.

Seek urgent medical care for severe or one-sided abdominal pain, shoulder-tip pain, fainting, marked dizziness, difficulty breathing, fever, or bleeding that is rapidly soaking pads. These signs require assessment for significant blood loss, infection or an ectopic pregnancy.

Contact your clinician promptly if bleeding or pain is worsening, pregnancy tests remain positive as follow-up continues, or you are unsure when the next hCG test or ultrasound is due.

You could ask whether your hCG needs repeating, whether an ultrasound would clarify the pregnancy location, what bleeding pattern to expect, and which pain medicine suits you. Also ask how you will know follow-up is complete and whom to contact if symptoms change.
