What to Know About a Big Sebaceous Cyst

Do not squeeze, puncture, or try to cut it out yourself. A clinician can confirm whether the lump is a cyst and discuss observation, treatment for inflammation, drainage when appropriate, or removal.

The term sebaceous cyst is commonly used for an epidermoid cyst: a closed sac beneath the skin filled with keratin, the material found in the outer skin layer. It may feel round and movable, and a small central opening can sometimes be visible.

Size alone does not show whether a lump is infected or identify it with certainty. Rapid enlargement, tenderness, redness, warmth, unpleasant-smelling discharge, or skin breakdown can signal inflammation or infection. Other skin lumps can resemble a cyst, so an examination matters when the diagnosis is unclear.

Keep the area clean, reduce rubbing from clothing or equipment, and use a warm compress for comfort if the skin is intact. If fluid drains on its own, cover it with clean gauze and wash your hands before and after touching the dressing.

Book a routine appointment for a large, persistent, or bothersome lump. A clinician may recommend monitoring a quiet cyst. An inflamed cyst may be treated differently from an infected collection, while definitive removal generally involves taking out the cyst wall after acute inflammation has settled. Simple drainage can relieve pressure but may leave the cyst lining behind, allowing the lump to return.

Arrange prompt medical assessment if the lump becomes increasingly painful, hot, red, swollen, or starts releasing pus; if redness is spreading; or if you feel feverish or generally unwell. These features can accompany infection and may require treatment rather than home care.

Seek urgent help if swelling interferes with breathing or swallowing, is close to the eye with worsening swelling or vision changes, or is accompanied by severe illness. Also have the lump examined if it is hard and fixed, grows quickly, repeatedly bleeds, develops an open sore, or does not behave like a previously diagnosed cyst.

Ask what features support the diagnosis, whether inflammation or infection is present, and whether examination alone is sufficient. If treatment is offered, ask whether it aims to calm inflammation, drain a collection, or remove the entire cyst wall; what wound care will be needed; and how the cyst's location may affect scarring or movement.
