Big hives: what they mean and when to get help

Their size can look alarming, but urgency depends on associated symptoms rather than the diameter of a welt. Breathing difficulty, throat tightness, faintness, or swelling of the tongue or inside the mouth requires emergency help.

Individual welts are often smooth, raised, and intensely itchy. They may be pink, red, or close to the surrounding skin colour. One mark can fade while another appears elsewhere; nearby welts may join into broad irregular patches. This shifting pattern supports hives more than a mark that stays fixed and leaves bruising or scaling.

Triggers can include infection, food, medicine, insect stings, heat, cold, pressure, or exercise, although no clear trigger is found in many episodes. Deeper swelling of eyelids, lips, hands, or feet can accompany hives and is called angioedema.

Move to a cool environment, wear loose clothing, and use a cool compress. Avoid scratching and pause any suspected non-essential skin product. A pharmacist or clinician can advise whether a non-drowsy oral antihistamine is suitable, particularly if you are pregnant, have another medical condition, or take other medicines.

Record foods, new medicines, supplements, infections, stings, exercise, temperature exposure, and pressure on the skin before the episode. Do not stop a prescribed medicine without medical advice unless emergency services direct you during a severe reaction.

Call emergency services now if hives occur with trouble breathing, wheezing, throat tightness, a hoarse voice, tongue or mouth swelling, collapse, confusion, or faintness. Use a prescribed adrenaline auto-injector without delay if your emergency plan tells you to, then seek emergency care.

Arrange clinical review when episodes keep returning, do not settle, disrupt sleep, begin after a new medicine, or involve painful marks that remain in one place or leave bruising. Photographs can help if the welts disappear before the appointment.

Ask which antihistamine and dose fit your health history, whether the pattern suggests a physical trigger such as pressure or heat, and what symptoms belong in an emergency plan. Bring a medicine list and photographs showing how quickly the welts change.
