Big dandruff flakes: causes, care and warning signs

Flakes can clump together when shed skin mixes with scalp oil or remains trapped among the hair. Dandruff typically presents with loose scalp scale and may include itching. Seborrheic dermatitis commonly presents with more visible inflammation and greasy or yellowish scale, sometimes around the hairline, ears, eyebrows or sides of the nose. Thick sharply defined patches, broken hairs, marked tenderness or discharge are not features to dismiss as ordinary flaking and merit examination.

Loosen scale during washing with your fingertips and avoid scraping with nails or a comb. A medicated shampoo may help when selected for the likely cause; apply it to the scalp rather than only the hair, follow its label for contact time and rinse thoroughly. Different active ingredients target scale, inflammation or yeast, so a pharmacist or dermatologist can help choose one that suits your scalp and hair. Stop a product and seek advice if it causes strong burning, swelling or a widespread rash.

Book a clinical review if careful shampoo use does not control the scale, if symptoms repeatedly return, or if the diagnosis is uncertain. Seek earlier care for painful swelling, pus, crusting, spreading redness, patchy hair loss or fever. Sudden facial swelling, breathing difficulty or a rapidly spreading rash after a hair product requires urgent help. These decisions are based on the accompanying scalp and whole-body signs, not the size of a flake by itself.

Tell the clinician whether scale is dry or greasy, where else it appears, how much it itches and which washes or styling products touch your scalp. Ask which diagnosis fits the distribution, how to alternate medicated and gentle shampoos, and whether a topical antifungal treatment is appropriate. Mention broken hairs, recent hair treatments and any reaction that started after a new dye or oil.
