The short answer
The abdomen has been stretched by pregnancy and affected by surgery, fluids, reduced movement and slower bowel activity. The shape does not return immediately after birth. What matters is whether swelling is gradually settling or becoming more tense, one-sided, painful or associated with wound changes.
Why your belly may be swollen
Normal healing can leave puffiness around the incision, while trapped wind and constipation may make the whole abdomen feel full. The uterus also needs time to contract after delivery, and stretched abdominal muscles provide less support during early recovery. These effects can overlap.
Swelling concentrated at the wound deserves closer attention if it is getting hotter, redder, more painful or begins leaking fluid, because infection or a collection beneath the skin may need treatment. A new bulge that is more obvious with coughing or standing also merits examination. General swelling with headache, visual disturbance or pain beneath the ribs requires urgent assessment for a postnatal blood-pressure problem, including pre-eclampsia.
How to support recovery
Follow the wound-care instructions from your maternity team, wash your hands before touching the area, and keep the incision clean and dry. Wear loose clothing that does not rub the scar. Gentle walking can support circulation and bowel movement, but build activity gradually and avoid lifting beyond the limits your team gave you.
Drink regularly and include fibre-containing foods if constipation is contributing, unless your clinician advised otherwise. Take prescribed pain relief as directed. Do not massage a fresh incision or start demanding abdominal exercises without clearance. Once healing is established, postnatal physiotherapy can assess breathing, movement, scar comfort, abdominal muscle function and pelvic-floor recovery.
When swelling needs urgent care
Contact your maternity team promptly if the wound becomes increasingly swollen, hot, red, painful, smelly, opens or releases pus or fluid. Seek urgent care for severe or rapidly worsening abdominal pain, heavy vaginal bleeding, fever, repeated vomiting, faintness or feeling seriously unwell.
Call emergency services for sudden breathlessness, chest pain, coughing blood or collapse. New pain or swelling in one calf also needs urgent assessment for a blood clot. Severe headache, vision changes, upper abdominal pain, sudden swelling of the face or hands, or breathing difficulty after birth should be assessed without delay.
What to ask at your review
Ask whether the pattern looks consistent with wound healing, bowel distension, fluid retention or muscle separation; how to check the scar safely; and when to increase walking, lifting and abdominal exercise. Mention worsening pain, altered bladder or bowel function, unusual discharge and any emotional concerns about recovery.
