The nickname is misleading because injections are usually placed around the upper shoulders and neck rather than throughout the arms. This cosmetic use may be outside the product's approved indications, and weakening a muscle involved in posture and shoulder movement has functional as well as aesthetic consequences.

Botulinum toxin type A temporarily reduces nerve signals that make a targeted muscle contract. Less trapezius activity may soften a prominent upper-shoulder outline over time, but it does not remove fat, tighten loose skin or reshape bone.

Shoulder contour differs with skeletal structure, muscle size, posture and body composition. An injector should assess neck and shoulder strength, asymmetry, occupation, exercise demands and any history of nerve or muscle disease before discussing whether the desired change is realistic.

Choose a licensed clinician trained in anatomy and the management of botulinum toxin complications. Ask which product is being used, whether this site is an approved use, how placement is planned, and what support is available if weakness develops.

Disclose pregnancy or breastfeeding, swallowing or breathing problems, neuromuscular conditions, previous toxin reactions and all medicines or supplements. Local tenderness, bruising, headache or uneven shoulder appearance can occur. Excess weakening may affect shoulder elevation, neck comfort, posture, lifting or exercise. Contact the treating clinician if new weakness interferes with ordinary movement or asymmetry is pronounced.

Seek emergency care for difficulty breathing, swallowing or speaking; widespread weakness; loss of bladder control; double vision; or signs of a severe allergic reaction such as facial swelling. These symptoms require assessment rather than waiting for the toxin's effect to fade.

Ask how your shoulder function will be documented, what outcome is plausible for your anatomy, which activities could be affected, and whom to contact outside clinic hours. Clarify the plan if one side responds differently and whether non-injectable approaches better match the concern.
