Bad posture and chest tightness: what to know

Short answer
Bad posture may tighten chest and upper-back muscles, yet new chest tightness should not be assumed to be muscular.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

- Emergency clue
- Pressure with breathlessness, sweating or spreading pain
Possible explanations
Rounded shoulders and prolonged sitting can alter the load on the chest wall, neck and upper back. This may cause a pulling sensation, tenderness or discomfort that changes with movement or position. Chest tightness can also occur with asthma or COPD, acid reflux, anxiety or panic. Angina is another possible cause, particularly when pressure comes with exertion or spreads to the arm, jaw, back or shoulder. The sensation and its triggers matter more than posture alone, and a remote description cannot safely identify the source.
What to do
Change position regularly, support your back, relax your shoulders and take gentle movement breaks. Stop an activity that brings on tightness. Record whether symptoms follow meals, exertion, stress, a particular posture or exposure to a trigger. Do not borrow an inhaler or rely on stretching to explain away new symptoms. Arrange medical advice for recurring, unexplained or activity-related tightness; assessment may include heart and lung checks.
When to seek urgent help
Call emergency services for severe or persistent tightness, pressure spreading to the arm, jaw or back, breathlessness, fainting, sweating, blue lips, or sudden weakness. Seek prompt care for coughing blood, a new irregular heartbeat, or symptoms that begin after exertion. People with known heart or lung disease should follow their personalised emergency plan.
Questions for your doctor
Could the pattern fit a chest-wall strain, reflux, asthma or a heart problem? What examination or tests are appropriate? Should I change my workstation or activity? If I use inhaler therapy, when should I use it and when is urgent care needed?
