Back flank pain on the left side: what to know

The left flank lies between the lower ribs and the hip. A pulled muscle can cause soreness that changes with twisting, bending, or touch. Kidney infection commonly presents with flank pain plus fever, chills, nausea, or painful and frequent urination. Kidney stones can cause intense, cramping pain that comes in waves and may travel toward the groin. A urinary tract infection can bring burning urination and urgency; flank pain or fever suggests the infection may need prompt review.

Record when the pain began, whether it is steady or wave-like, and whether movement affects it. Avoid strenuous lifting while you observe it, keep gently mobile, and use heat only if it feels soothing. Seek a clinician’s advice if it does not settle, keeps returning, or is paired with urinary symptoms. Testing may include an examination and urine testing, with further assessment when indicated.

Use urgent services for severe pain, fever or chills, repeated vomiting, fainting, confusion, inability to urinate, or blood in the urine. Arrange prompt care after a blow to the back, or if you are pregnant or have kidney disease. A new symptom that is steadily worsening deserves timely evaluation even without urinary symptoms.

Ask whether the pain seems muscular, urinary, or related to another nearby structure; whether urine testing is needed; and what symptoms should trigger reassessment. Tell the clinician about bowel symptoms, fever, urinary changes, recent activity, injuries, and all medicines you take.
