The short answer

Feeling well does not reveal whether a person has been infected. Tuberculosis screening can identify an immune response, while symptom review, chest imaging and tests on sputum or other samples help clinicians look for active disease.

Why there may be no warning signs

In latent infection, the immune system contains the bacteria. There is no cough, fever, night sweating or weight loss, and the person does not pass TB to others. The bacteria can remain in the body and may become active if immune control weakens.

Active pulmonary tuberculosis often presents with a persistent cough and can also cause fever, night sweats, poor appetite, weight loss, fatigue or blood-stained sputum. Tuberculosis outside the lungs presents according to the body area involved, sometimes with swollen glands or local pain.

When screening is worth discussing

Speak with a clinician about screening after close exposure to someone with infectious TB or when work, travel, residence, a planned immune-suppressing treatment or a health condition makes assessment relevant. HIV infection can weaken the immune response, so tailored testing and follow-up are particularly important.

After a positive screening result, attend the recommended examination and chest X-ray even without symptoms. Do not assume the result proves active disease. If latent infection is diagnosed, discuss preventive medicine, interactions with current treatments and the monitoring plan.

Changes that warrant medical review

Arrange prompt assessment for a cough that persists, recurrent fever, drenching night sweats, unexplained weight loss, worsening fatigue or new swollen glands, particularly after known TB exposure. These symptoms have many possible causes, and examination determines the next tests.

Urgent medical care is needed for coughing up blood, major breathing difficulty, severe chest pain, confusion or collapse. If active lung TB is suspected, follow the clinical team's advice about masks, ventilation and close contact while tests are underway.

What to ask about testing

Ask which screening method suits you, whether a previous TB vaccine or earlier treatment affects interpretation, and when testing should occur after exposure. If a result is positive, clarify how active disease will be excluded, whether household members need assessment, and who to contact if symptoms appear.

Questions people ask

Does no cough mean tuberculosis is ruled out?

The absence of a cough does not rule out tuberculosis. Latent infection has no symptoms, and active TB outside the lungs may not cause a cough; testing is guided by exposure, health factors and clinical findings.

Can latent tuberculosis become active later?

Bacteria held under immune control can reactivate, especially when immunity becomes weaker. Preventive treatment may be offered after active disease has been excluded.

Is a chest X-ray enough to diagnose tuberculosis?

A chest X-ray alone cannot diagnose tuberculosis. It can show changes that support further investigation, but clinicians interpret it with symptoms, screening results and laboratory tests.
