How aortic stenosis can lead to heart failure

Over time, the main pumping chamber may thicken, stiffen or weaken. Pressure can then build behind the heart and fluid may collect in the lungs or legs. Timely assessment matters because symptoms and heart function help clinicians decide how closely to monitor the valve and whether specialist treatment is needed.

The aortic valve controls blood flow from the left side of the heart into the aorta. When its opening narrows, the heart must generate more pressure to move blood forward. Heart failure describes the resulting inability to fill or pump effectively enough for the body's needs; it does not mean the heart has stopped.

Aortic stenosis may present with breathlessness on exertion, chest pressure, fainting or reduced exercise capacity. When heart failure develops, people may also notice fatigue, difficulty breathing when lying flat, waking breathless, swollen ankles, rapid weight gain from fluid or a persistent cough.

Report new or changing symptoms rather than waiting for a routine visit. Take prescribed medicines exactly as directed, but understand that medicines used for heart failure do not remove the valve narrowing itself. Keep appointments for examination and heart imaging, and bring an updated medicine list.

Ask your clinician what activity is appropriate for the severity of your valve disease. Track swelling, breathlessness and changes in weight if advised. Do not abruptly change fluid tablets, blood-pressure treatment or fluid intake without clinical guidance, as valve narrowing can affect how the circulation responds.

Call emergency services for severe breathing difficulty, chest pain that does not settle, fainting, blue or grey lips, confusion, or sudden marked weakness. These features can reflect reduced blood flow or fluid in the lungs and need immediate assessment.

Contact your heart team or usual clinician promptly for increasing breathlessness, new ankle swelling, needing more pillows to breathe comfortably, waking short of breath, palpitations or a noticeable fall in activity tolerance. Symptoms should guide urgency even if a routine review is already booked.

Ask how severe the valve narrowing is, whether heart function has changed, which symptoms you should track, and when imaging should be repeated. It is also reasonable to discuss whether a valve procedure is being considered, how your other conditions affect that decision, and whom to contact if symptoms worsen.
