The different parts of the picture

Functional impact covers movement, sleep, work, self-care, and fatigue. These dimensions can move in different directions and require different responses.

Clinicians build the picture from your symptom pattern, physical examination, inflammatory markers when useful, and selected imaging. They may repeatedly check spinal mobility, chest expansion, hip movement, and the effect of symptoms on ordinary tasks. A comparison over time is often more informative than a single score or scan.

How to track meaningful change

Keep brief notes on morning stiffness, night waking, fatigue, exercise tolerance, medicine use, and activities you have stopped or modified. Mention heel pain, swollen joints, eye inflammation, bowel symptoms, or skin changes because ankylosing spondylitis can affect more than the back.

Continue the agreed treatment even when a broad stage label feels unclear. Management may include anti-inflammatory medicine, targeted disease treatment, regular exercise, and physiotherapy. Reviews are a chance to adjust treatment to current activity and goals rather than waiting for a supposed next stage.

Changes needing faster review

A painful red eye with sensitivity to light or blurred vision needs urgent eye assessment. Emergency care is needed for new bladder or bowel dysfunction, numbness around the groin, marked limb weakness, or severe spinal pain after trauma.

Bring a routine review forward if stiffness, fatigue, or night pain begins to limit daily life despite treatment, or if medicine causes concerning effects. A change in symptoms may reflect active disease, a treatment issue, or a separate condition; it does not automatically mean progression to an advanced stage.

If your clinician uses a stage description, ask whether it refers to inflammation, imaging changes, or functional limitation. Find out what will be monitored, which findings would change treatment, and what you can do to protect mobility, bone health, posture, and participation in work or sport.
