How ankylosing spondylitis can begin

Short answer
Ankylosing spondylitis often begins gradually with inflammatory pain and stiffness around the lower back or buttocks.
This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.
At a glance
- Typical starting areas
- Lower back, sacroiliac region, or buttocks
- Helpful observation
- Compare symptoms after rest with symptoms after movement
- Relevant specialist
- Rheumatologist
The early pattern
Discomfort may alternate between the buttocks, disturb sleep, feel worse after rest, and ease after getting moving. Symptoms can come and go, so an early flare may be mistaken for a strain. Some people first notice pain where tendons attach to bone, such as the heel, or swelling in a hip, knee, or ankle.
What onset means
Onset describes when the symptom pattern starts, not the date on which the diagnosis is confirmed. Ankylosing spondylitis is an inflammatory form of axial spondyloarthritis that mainly involves the spine and sacroiliac joints, where the pelvis meets the lower spine.
No single symptom confirms it. A clinician considers the pattern over time, examines spinal and joint movement, and may use blood tests and imaging. Early imaging can be normal, so a normal result does not automatically settle the question if an inflammatory pattern continues.
Prepare for an assessment
Record where pain begins, whether it wakes you, how stiffness changes with rest or activity, and whether you have heel pain, a red eye, psoriasis, or ongoing bowel symptoms. Include any close family history of axial spondyloarthritis, psoriasis, or inflammatory bowel disease.
Arrange a routine medical review when back or buttock pain keeps returning, morning movement is restricted, or activity consistently helps more than rest. Continue comfortable daily movement, but avoid forcing a stiff spine through sharp pain. A physiotherapist can tailor mobility, posture, breathing, and strengthening work after assessment.
When care should be urgent
Seek urgent same-day eye assessment for a painful red eye, marked light sensitivity, or blurred vision because ankylosing spondylitis can occur with uveitis. Do not wait for a routine arthritis appointment.
Go to emergency care after a significant fall or impact if there is new severe spinal pain, weakness, numbness around the groin, loss of bladder or bowel control, chest pain, or serious breathing difficulty. These symptoms require prompt evaluation regardless of the cause.
Useful questions for your clinician
Ask which features suggest inflammatory rather than mechanical back pain, whether a rheumatology assessment is appropriate, and what examination or imaging would add. Also ask which activities are suitable during a flare and how treatment response will be monitored.

Questions people ask
Can ankylosing spondylitis start suddenly?
The underlying pattern is often gradual, although an individual flare can feel abrupt. A clinician should assess sudden severe pain separately, especially after an injury.
Does morning stiffness prove the diagnosis?
No. Ankylosing spondylitis commonly presents with morning stiffness, but other conditions can cause it. Diagnosis depends on the wider history, examination, and selected tests.
Why can movement feel better than rest?
Inflammatory stiffness may become more noticeable during inactivity. Gentle movement can loosen the spine and joints, although exercise alone cannot establish the diagnosis.
