Angular cheilitis during pregnancy

Pregnancy does not make every mouth-corner crack pregnancy-related. Saliva irritation, lip licking, dry lips, vomiting, dentures, eczema, yeast or bacteria can contribute. Iron or other nutritional deficiencies can also cause mouth changes, so recurrent soreness may warrant assessment rather than repeated self-treatment.

Angular cheilitis produces redness or altered colour, tenderness and splits where the upper and lower lips meet. Opening the mouth can reopen a fissure. A clinician may look for thrush inside the mouth, honey-coloured crusting, a contact reaction, dental or denture issues, and signs of anaemia or another deficiency. This examination helps distinguish simple irritant inflammation from an infection needing an antifungal or antibiotic.

Gently rinse away food or saliva, pat the corners dry and apply a simple fragrance-free barrier ointment. Avoid licking, picking and newly introduced flavoured lip products. Maintain regular meals and take pregnancy supplements only as directed; extra iron or vitamins should follow advice because the right dose depends on your needs. Tell a pharmacist, midwife or doctor that you are pregnant before using an antifungal, antibiotic or corticosteroid near the mouth. Some topical options may be suitable, but the active ingredient, strength, location and duration all matter.

Request a routine review when the crack is not settling, keeps returning, affects eating, or comes with fatigue, a sore tongue, white patches in the mouth or other skin changes. Seek urgent care for rapidly spreading redness, increasing facial swelling, severe pain, fever, pus, difficulty swallowing, or breathing problems. Contact your maternity team if vomiting or mouth pain prevents you from keeping down fluids, since dehydration needs attention during pregnancy.

Ask what cause is most consistent with the examination, whether a swab is useful, which barrier has the fewest irritating ingredients, and whether the recommended medicine is appropriate at your stage of pregnancy. If symptoms recur, discuss whether a blood count, iron assessment, diet review, oral examination or denture check would help.
