Angioedema around one eye: what to check

One-sided puffiness does not by itself confirm the cause. Check for itching, hives, redness, tenderness, discharge, an insect sting, recent skin injury, and swelling elsewhere.

Eyelid tissue is loose, so deeper fluid swelling can become prominent and may close the eye without damaging the eyeball. Angioedema is often soft or tight rather than sharply painful and may accompany lip swelling or hives.

A tender red eyelid with fever, worsening warmth, reduced vision, a bulging eye, or pain when looking around needs assessment for infection or disease behind the eye. Local allergy, a blocked eyelid gland, injury, and insect bites can also resemble angioedema.

Remove contact lenses, avoid rubbing, and place a clean cool compress over the closed lid without pressure. Do not apply a new cosmetic, essential oil, or medicated cream close to the eye. Take a photograph to document whether the swelling is spreading.

Follow a clinician's previous plan if this is a familiar episode. A recommended antihistamine may help when itching or hives indicate a histamine-related reaction, but treatment depends on the cause and may be different for other forms of angioedema.

Get urgent medical care for eye pain, fever, marked redness, visual change, double vision, a protruding eye, severe headache, vomiting, or pain with eye movement. These features require an eye and infection assessment rather than assuming angioedema.

Call emergency services if eyelid swelling occurs with tongue or throat swelling, breathing difficulty, hoarseness, faintness, or rapidly spreading facial swelling. New unexplained one-sided swelling should also be reviewed if it persists or worsens.

Describe the order in which symptoms appeared and ask whether infection, contact allergy, or angioedema fits the examination. Mention eye products, medicines, bites, injuries, sinus symptoms, and earlier episodes, and ask which future changes should prompt same-day care.
