Common locations

People may describe a broad pressure, squeezing, weight, burning or tightness rather than pain in one precise spot. Some presentations involve breathlessness or fatigue with little obvious chest discomfort.

Why discomfort can travel

Signals from the heart enter the nervous system near signals from other upper-body areas. The brain may therefore perceive heart distress in a shoulder, arm, jaw or back; this is called referred discomfort.

Location is only one part of the pattern. Angina is more characteristically linked to increased cardiac demand, such as physical effort or emotional stress, and may settle with rest or prescribed treatment. Indigestion, muscle strain and lung problems can overlap in location, so evaluation considers triggers, sensation, associated symptoms and examination findings.

Describe the episode clearly

When it is safe to do so, note the starting point, spread, trigger, duration and relief. Mention breathlessness, sweating, nausea, dizziness or weakness, and whether pressing or moving the area recreates the discomfort.

Recurring symptoms need clinical review even when they settle. Your clinician may use an electrocardiogram and other tests to assess the heart while also considering digestive, muscular and respiratory explanations.

Urgency depends on the whole pattern

Call the local emergency service for new or severe pressure anywhere in the chest or upper body when it occurs at rest, persists, or comes with breathlessness, sweating, nausea, faintness or collapse. Spreading discomfort and a sudden change in established angina are also emergency warning signs.

Do not dismiss symptoms because they are on the right side, high in the abdomen or away from the chest. Arrange emergency transport rather than driving when red flags are present.

Can angina feel like indigestion?
