Antibiotics for amoebiasis: what patients should know

Treatment may involve a medicine that acts in body tissues followed by another that clears organisms remaining inside the bowel. A clinician should choose the drugs because ordinary antibiotics do not all work against the parasite that causes amoebiasis.

Entamoeba histolytica can invade the bowel wall and may occasionally affect the liver. A tissue-active medicine targets invasive infection, while a bowel-active medicine helps remove parasites left in the intestine. The combination and sequence matter, even if diarrhoea settles early.

The word antibiotic is often used broadly here, but the prescribed drugs are selected specifically for an amoebic infection. A stool assessment, travel history, symptoms and examination can help distinguish amoebiasis from other causes of bowel upset.

Take each dose at the instructed time and complete every part of the regimen unless the prescriber changes it. Tell the clinician about pregnancy, breastfeeding, liver problems, allergies and all medicines or supplements you use. Ask what to do after a missed dose rather than doubling it.

Drink enough safe fluid to replace losses from loose stools. Wash hands with soap after using the toilet and before preparing food, and avoid preparing meals for others while you have diarrhoea. Do not share leftover tablets or use another person's prescription.

Seek urgent assessment for blood in the stool, severe or worsening abdominal pain, repeated vomiting, faintness, confusion, very little urine, or difficulty keeping fluids down. Fever with pain under the right ribs also needs prompt review because infection outside the bowel is possible.

Contact the prescriber if symptoms worsen during treatment, return after the course, or a medicine causes swelling of the face, breathing difficulty, a widespread rash, marked weakness or persistent vomiting. Breathing difficulty or facial swelling warrants emergency help.

Ask whether testing has confirmed Entamoeba histolytica, whether you need both tissue-active and bowel-active treatment, which side effects merit a call, and whether follow-up testing is appropriate. Also clarify food or alcohol restrictions and how the plan fits your other medicines.
