Patterns that help explain body aches

Body aches may feel like soreness, heaviness, tenderness or stiffness across several muscle groups. A new workout, physical work, poor sleep or prolonged immobility can produce a clear mechanical pattern. Viral illnesses such as influenza and COVID-19 commonly present with muscle aches alongside fever, fatigue, cough or throat symptoms.

Other possibilities depend on context. Dengue fever or malaria can present with marked aches and fever after relevant travel or mosquito exposure, while brucellosis can include aches after contact with infected animals or unpasteurised dairy products. Longer-lasting widespread pain may occur with fibromyalgia, hypothyroidism or long COVID. A symptom alone cannot identify which explanation applies.

Practical care while you monitor symptoms

Rest from strenuous activity, drink enough fluid to keep urine a pale colour, and try gentle movement if stiffness improves with activity. Heat may soothe tense muscles. If you use a non-prescription pain medicine, check that it is suitable for your age, pregnancy status, medical conditions and current medicines.

Note when the aches started and whether fever, rash, cough, weakness, joint swelling, dark urine, travel, a tick or mosquito exposure, a new medicine or an unusually hard workout occurred. Arrange a routine appointment when pain persists, repeatedly returns, disrupts sleep or daily function, or comes with ongoing fatigue or unexplained weight change.

Red flags that change the urgency

Get urgent help for trouble breathing, chest pain, confusion, fainting, a stiff neck with fever, a seizure, new one-sided weakness, inability to walk, severe dehydration, or a purple or non-fading rash. Severe muscle pain with swelling, profound weakness or dark brown urine also needs prompt assessment.

Seek same-day advice for high or worsening fever after recent travel, significant immune suppression, pregnancy with systemic illness, or pain that began soon after starting a new medicine. Urgency comes from these warning features, not from any single cause being common.

Ask whether the pattern points to muscles, joints or nerves, whether infection testing or blood tests would change care, and which activities are safe to continue. Bring a medicine list and describe recent travel, exposures, exercise changes and every accompanying symptom.
