Bleeding after sex: causes and when to seek care

A small smear may follow dryness, vigorous contact or minor surface irritation. Cervical inflammation, a cervical polyp, chlamydia and thinning vaginal tissue around or after menopause are other possibilities. Cervical or vaginal cancer can present with bleeding after sex, but the symptom alone does not identify the cause.

Notice whether blood appears only on wiping or continues like menstrual flow. Pelvic pain, unusual discharge, odour, itching, pain during sex, missed periods or bleeding between periods provide useful clues. Blood can also come from a partner, the urinary tract or the rectum rather than the vagina.

Pregnancy status, contraception, menopause, cervical screening history and recent procedures shape the assessment. A clinician may examine the vulva, vagina and cervix, and may offer pregnancy testing, infection swabs or cervical testing depending on the findings.

Pause penetrative sex if it triggers pain or fresh bleeding. Use a pad rather than inserting a tampon while monitoring the amount, and avoid vaginal washes that could worsen irritation. Arrange a medical appointment if bleeding happens again, persists, occurs after menopause or accompanies unusual discharge.

If an infection is possible, testing is more useful than self-treating. Until assessed, barrier protection can reduce exposure between partners. Keep a note of the bleeding pattern and the first day of the last menstrual period.

Seek urgent care for heavy or continuing bleeding, severe pelvic or abdominal pain, fainting, marked dizziness, weakness or shoulder-tip pain. If pregnancy is possible, bleeding with pain or light-headedness needs prompt assessment because a pregnancy complication must be excluded.

Get timely medical review for bleeding after menopause even when it is light, and for recurrent postcoital bleeding without pain. These patterns warrant examination; they do not by themselves mean cancer.

Ask whether the cervix and vagina need examination, whether pregnancy or infection testing is appropriate, and when cervical screening is due. Clarify when sexual activity can resume comfortably and what change in bleeding should prompt urgent help.
