The short answer

Pregnancy-related fluid, intravenous fluids given around surgery, and reduced movement can all contribute. Swelling should be considered alongside pain, skin colour, breathing, blood-pressure symptoms, and the condition of the incision.

Where swelling can appear

Even puffiness in both lower legs is often fluid retention as the body recovers after pregnancy. Localised fullness and mild tenderness around the incision may accompany healing, while increasing redness, heat, drainage, or separation of the wound suggests a complication that needs review.

One calf or thigh that becomes swollen, warm, red, or painful can be a sign of a blood clot. Sudden swelling of the face or hands with headache or visual changes can accompany postpartum high blood pressure.

Ways to manage mild swelling

Take short, gentle walks as permitted, change position regularly, rest with your feet supported, drink water, and follow the wound-care instructions from your maternity team. Avoid massaging a painful or one-sided swollen leg.

Check both legs and the incision each day. Note whether swelling is symmetrical, whether pressing leaves a dent, and whether redness, pain, discharge, headache, or breathlessness develops. Do not start a water tablet unless it has been prescribed for you.

Warning signs after birth

Get urgent help for sudden breathlessness, chest pain, coughing blood, collapse, or a painful swollen leg. These symptoms may signal a clot in the leg or lungs and should not be watched at home.

Contact maternity care promptly for severe headache, vision changes, pain below the ribs, vomiting, sudden facial or hand swelling, or a high blood-pressure reading. Fever, worsening wound pain, spreading redness, pus, bad odour, or an opening incision also requires timely examination.

What to discuss at follow-up

Describe exactly where the swelling began and what makes it change. Ask whether your blood pressure, incision, and legs need examination, which movements are suitable now, and whether any prescribed medicine could contribute to fluid retention.
