Symptoms of acute labyrinthitis

The spinning may worsen with movement, but it is often present even when the head is still during the early phase.

The inner ear supplies the brain with balance information. When one side becomes inflamed, mismatched signals can create a strong illusion that you or the room is moving. Walking may veer to one side, and eye movements can add to nausea and difficulty focusing. Hearing may become muffled, and ringing can occur in the affected ear.

Symptoms often begin abruptly and can remain intense before gradually easing. Residual imbalance may outlast the severe spinning. Vestibular neuritis has a similar balance pattern but usually lacks new hearing symptoms; brief attacks brought on only by particular head positions suggest a different balance disorder.

Stop walking when spinning surges and sit or lie somewhere secure. Move the head slowly, use support when standing, improve lighting and remove trip hazards. Do not drive, cycle, operate machinery or swim alone until balance and vision feel dependable.

A clinician should assess a new severe or persistent episode rather than relying on symptoms alone. Note the start time, hearing changes, ear pain, headache, recent infection, vomiting and any weakness, numbness or speech difficulty. This history helps distinguish an inner-ear problem from causes requiring emergency treatment.

Get emergency help for vertigo accompanied by face or limb weakness, numbness on one side, slurred speech, confusion, new double vision, collapse, a sudden severe headache or inability to coordinate walking. Arrange urgent same-day assessment for sudden hearing loss, repeated vomiting, dehydration, high fever, severe ear pain or rapidly worsening symptoms.

Ask which examination findings point to the inner ear, whether a hearing test is appropriate, and what alternative diagnoses have been considered. Clarify how long to use symptom-relief medicine, what movements or activities to avoid temporarily, and when persistent imbalance should lead to vestibular rehabilitation or specialist assessment.
