The short answer

It often happens during sprinting, kicking or a rapid stretch. Pain may be sharp and can be accompanied by weakness, bruising or difficulty walking. The amount of lost function matters more than trying to grade the injury yourself.

What the injury means

The hamstrings cross the hip and knee, helping extend the hip and bend the knee. A tear can affect muscle fibres, the junction between muscle and tendon, or the tendon itself. A clinician checks the injury location, strength, walking pattern and the circumstances of the sudden pain. Imaging may be considered when a major tear, tendon detachment or another injury is suspected.

Bruising can track down the thigh after the initial injury. Its size does not by itself show exactly how much tissue is torn.

What to do now

Stop the activity that caused the pain and avoid forceful stretching, sprinting or deep massage during the early painful period. Relative rest, a wrapped cold pack for short comfortable intervals and elevation may help symptoms. Do not place ice directly on skin. If walking is difficult, reduce weight through the leg and arrange an assessment.

Recovery usually progresses from comfortable movement to strengthening, faster running and sport-specific drills. Advancing because pain has settled, before strength and control return, can overload healing tissue. A physiotherapist can tailor loading and return-to-sport tests to your activity.

When to seek medical care

Seek prompt assessment if you felt a pop and cannot walk normally, have marked weakness, rapidly increasing swelling, extensive bruising, a visible hollow in the muscle, or pain close to the buttock after a forceful injury. These features can accompany a substantial tear or tendon injury.

Get urgent help if the foot becomes cold, pale, numb or increasingly weak, or if severe swelling and pain continue to escalate. Arrange a routine review when symptoms are not steadily improving or repeatedly return during rehabilitation.

Avoid forceful stretching while the injury is acutely painful. Gentle movement within a comfortable range may be introduced, then progressed according to symptoms and clinical guidance.

Does every hamstring tear need a scan?

When can I return to sport?
