Active keloid: signs and treatment options

How the structures involved in Keloid Scars differ from normal

Short answer

An active keloid is a raised scar that is still growing or causing symptoms such as itching, tenderness, tightness, or redness.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Typical feature
Raised scar tissue extending beyond the original injury
Possible symptoms
Itch, tenderness, tightness, or sensitivity
Treatment goal
Reduce bulk or symptoms while limiting renewed growth

The short answer

Unlike a scar that stays within the original wound, a keloid extends beyond its borders. Activity does not mean cancer; it describes continuing scar growth or inflammation-like symptoms.

What changes may occur

Keloids can follow surgery, piercings, burns, cuts, injections, or inflamed acne spots. They may feel firm or rubbery and can gradually spread beyond the site of the original injury. Colour varies with skin tone and the age of the scar.

Some remain mainly cosmetic, while others rub against clothing or restrict movement near a joint. A clinician may examine the shape, surface, border, and history to distinguish a keloid from a hypertrophic scar, cyst, or another skin lump.

What you can do

Avoid scratching, squeezing, home cutting, or unregulated injections. Reduce friction over the scar and protect it from strong sun, which can make colour contrast more noticeable.

Treatment is individual and may combine silicone-based care with clinician-delivered options such as injections, freezing, laser treatment, or surgery. Excision alone can be followed by renewed growth, so the dermatologist may discuss additional measures around the procedure. The aim may be flattening, symptom relief, easier movement, or reduced colour rather than complete disappearance.

When to arrange medical care

Book an assessment if the scar is enlarging, repeatedly painful, limiting movement, or causing troublesome itch. Prompt review is also sensible if the diagnosis has never been confirmed.

Seek quicker care for sudden bleeding without injury, an open sore that does not heal, pus, spreading warmth, fever, or rapid change in appearance. These findings are not explained simply by calling a scar active.

Useful questions at your visit

Ask how the diagnosis was confirmed, which symptom the plan is meant to improve, and whether several methods are likely to be combined. Discuss your previous response to wounds, the chance of colour change, discomfort control, and how recurrence will be monitored.

Questions people ask

Can an active keloid settle without treatment?

Its symptoms may fluctuate, but established keloid tissue does not behave like an ordinary fresh scar. Assessment can clarify whether observation or active management fits your situation.

Should I pierce through a keloid?

No. Further injury may worsen irritation or stimulate more scar tissue. Have the area assessed if jewellery is trapped or the site is sore.

Is surgery the only way to remove a keloid?

No. Injections, laser treatment, cryotherapy, and other approaches may be considered. Surgery is selected carefully because keloids can grow again after another skin injury.
