Acid reflux with black stool

Stool from bleeding higher in the digestive tract is often very dark, sticky or tar-like, and unusually strong-smelling. Iron tablets, bismuth-containing medicine, and some dark foods can also change stool colour, but that possibility should not delay care when the appearance is tarry or you feel unwell.

Acid reflux involves stomach contents travelling into the oesophagus and commonly causes burning or regurgitation. It does not ordinarily turn stool black. Bleeding in the oesophagus, stomach, or upper intestine can darken as it passes through the digestive tract, so having heartburn at the same time does not make black stool harmless.

Tell the clinician whether the stool is merely dark brown or truly black, whether it smears like tar, and whether there is red blood. Also report abdominal pain, vomiting, dizziness, weakness, fainting, breathlessness, or a racing heartbeat.

Obtain urgent medical advice for new black, tarry stool. If you are faint, confused, short of breath, vomiting blood, or too weak to travel safely, call emergency services and do not drive yourself. Keep the medication packets or a current list ready for the medical team.

Mention aspirin, anti-inflammatory painkillers, blood-thinning medicines, iron, bismuth remedies, supplements, alcohol use, and any history of ulcers or liver problems. Do not take an extra dose of reflux medicine to cover the symptom. Do not stop an essential prescribed blood thinner on your own; seek immediate instructions from the treating service.

Call emergency services for black tarry stool with collapse, faintness, confusion, clammy skin, marked weakness, breathing difficulty, severe chest or abdominal pain, vomiting blood, or vomit resembling coffee grounds. These features can accompany significant blood loss or another urgent illness.

Even without those additional signs, new tar-like stool warrants prompt clinical assessment. A clinician may check circulation, examine the abdomen, review medicines, and decide whether blood tests or examination of the digestive tract are needed.

Ask what is thought to have changed the stool colour, whether any medicine needs supervised adjustment, and which recurring signs should send you back for urgent care. Before leaving, confirm how follow-up will occur and how your reflux should be treated without obscuring warning symptoms.
