Aching elbow and forearm

Short answer

Aching around the elbow and forearm often reflects irritated tendons or muscles, but joint and nerve problems can produce a similar pattern.

This answers one question. It is not a diagnosis and does not replace an assessment by a licensed doctor.

At a glance

Outer elbow pattern
Often linked with wrist-extensor tendon loading
Inner elbow pattern
May involve flexor tendons or the ulnar nerve
Helpful observation
Which grip, wrist or elbow movement reproduces pain

The short answer

Pain on the outer elbow may accompany tennis elbow, while inner-elbow pain can occur with golfer's elbow. Stiffness and reduced movement may accompany elbow osteoarthritis, and tingling toward the ring or little finger can occur when the ulnar nerve is irritated at the elbow.

Location and triggers can separate the patterns

Tendon-related pain is often linked to repeated gripping, lifting, wrist work or racquet and throwing activities. Outer-elbow symptoms may worsen when lifting with the palm down; inner-elbow symptoms may be provoked by gripping or wrist flexion. Joint-related discomfort may feel deeper and come with grinding, swelling or loss of motion.

A nerve pattern deserves separate attention. Cubital tunnel syndrome can present with aching at the inner elbow plus tingling or numbness into the ring and little fingers, particularly while the elbow is bent. Examination helps distinguish these causes because more than one structure can be irritated at the same time.

Reduce irritation and preserve function

Modify the movement that repeatedly provokes the ache rather than stopping every arm activity. Use two hands for heavier objects, reduce forceful gripping, vary repetitive tasks and avoid resting the inner elbow on a hard surface. Gentle comfortable elbow and wrist movement can limit unnecessary stiffness.

If symptoms continue, a clinician or physiotherapist can test tendon loading, joint movement, grip and nerve function. Rehabilitation may include graded forearm strengthening and changes to work or sport technique. Injections and shockwave therapy are not interchangeable first steps; suitability depends on the diagnosis, prior care, expected benefit and individual risks.

When the ache needs medical review

Get urgent assessment after a forceful injury if the elbow looks deformed, will not move, or becomes rapidly swollen, or if the hand is cold, pale, blue or newly weak. A hot red joint with fever also needs prompt care.

Arrange a routine appointment when pain persists, repeatedly returns, disturbs sleep or limits grip and daily tasks. Book sooner for spreading numbness, progressive hand weakness, marked loss of elbow movement or swelling that is increasing rather than settling.

Prepare for a focused consultation

Describe whether the ache is inside, outside or deep in the joint and demonstrate the movement that triggers it. Ask whether the examination points to tendon, joint or nerve involvement, which activities can continue, what exercise progression is appropriate, and when a procedure would add value over rehabilitation.

Questions people ask

Does aching in the elbow and forearm always mean tennis elbow?

No. Tennis elbow is one possible outer-elbow pattern, while inner tendons, the elbow joint, nearby muscles and the ulnar nerve can cause different or overlapping symptoms.

Should I wear an elbow brace?

A brace may help selected tendon or nerve problems, but the type and placement depend on the suspected structure. Poorly fitted pressure can aggravate symptoms, so seek fitting guidance if the diagnosis is unclear.
