The short answer
It may begin when milk production, tissue swelling and narrowed milk ducts combine. Redness can be harder to see on brown or black skin, where warmth, swelling and tenderness may be clearer signs. Continuing to feed or express as comfortable is generally compatible with care.
How mastitis develops
Inflammation exists on a spectrum. Early breast fullness and a tender area do not always mean bacterial infection, but worsening systemic illness or symptoms that fail to settle can require medical assessment and antibiotics. A collection of infected fluid, called an abscess, may need imaging and drainage.
Mastitis commonly presents with one-sided breast pain, heat, swelling and a wedge-shaped or less clearly defined inflamed area. Fever, chills, headache and body aches can accompany it. Nipple damage, feeding difficulty or sudden changes in milk removal may contribute, but mastitis is not a sign of poor hygiene.
Care for the breast without overstimulation
Rest, drink according to thirst, use a cool compress, and consider a usual pain reliever that your clinician or pharmacist says is suitable for you. Feed responsively from either breast or express only enough for comfort if direct feeding is difficult. Abruptly stopping milk removal may worsen fullness.
Avoid deep massage, forceful squeezing, repeated extra pumping and attempts to empty the breast completely; these can aggravate swelling or drive excess milk production. Check pump-flange comfort and seek skilled feeding support for latch pain, nipple injury, oversupply or a feeding plan that is hard to sustain.
When to get medical help
Contact a clinician promptly if you feel increasingly unwell, fever or breast inflammation is worsening, symptoms are not improving with gentle measures, or mastitis returns. Seek urgent care for confusion, fainting, breathing difficulty, inability to keep fluids down, rapid deterioration, or signs of severe illness. A persistent or enlarging lump, soft fluid-filled area, or drainage needs examination for an abscess or another breast condition.
