Two C-sections and the possibility of a vaginal birth

A vaginal birth after two C-sections may be considered for some pregnancies, but it requires an individual obstetric assessment and a hospital prepared for an urgent caesarean. The decision is not based only on the number of previous births. Your obstetrician will review the uterine incision used previously, why the caesareans were needed, your recovery, the current pregnancy and your preferences.

A low transverse uterine scar may allow discussion of labour, while some other uterine incisions or a previous uterine rupture can make a planned caesarean more appropriate. The skin scar does not reliably show the uterine incision, so the operative notes matter. Placenta position, fetal position, pregnancy complications and whether labour starts on its own also shape the plan. A vaginal birth avoids abdominal surgery, whereas another caesarean offers a scheduled route but brings surgical recovery and scar-related considerations.

Arrange an obstetric review early enough to obtain both previous operation reports and discuss your preferred birth route. If labour is being considered, ask where continuous fetal monitoring is available and how quickly the team can perform an emergency caesarean. Agree on what to do if labour begins before the planned date, and discuss how induction could change the recommendation. Keep your maternity unit's contact instructions readily available.

Contact your maternity unit promptly if contractions begin, your waters break, vaginal bleeding occurs or your baby's movements are reduced. Seek emergency help for severe or persistent abdominal pain, heavy bleeding, fainting, breathing difficulty or a strong sense that something is wrong. Do not wait for a routine appointment when these symptoms occur, regardless of the planned method of birth.

Ask what your previous operative reports say about the uterine incisions, whether any current complication changes your options, and how your hospital manages labour after two caesareans. You can also ask about monitoring, pain relief, induction, the circumstances that would trigger surgery and expected recovery after either route.
