What it is

An anti-gravity treadmill is a treadmill enclosed by a waist-high air chamber. Once the chamber is sealed around the user, air pressure helps support part of the body weight. An anti-gravity treadmill reduces the load on your legs while allowing controlled walking practice during rehabilitation. The therapist can adjust support and speed so movement is challenging but manageable.

How it is used

A patient usually steps onto the belt, wears a fitted garment, and stands while the chamber seals around the waist. The therapist selects a support level, walking speed, session length, and hand support according to the rehabilitation goal. Training may begin with a shorter, easier walking pattern and progress as control, strength, and confidence improve. The therapist watches gait, balance, pain, and fatigue throughout the session.

Limits to know

Reduced loading can make walking practice easier, but it does not reproduce every demand of standing, stairs, uneven ground, or daily activity. The machine cannot correct a joint problem, heal tissue by itself, or replace strength and balance work. Some people may find the garment or chamber uncomfortable. The right support level depends on healing restrictions and should be set by the rehabilitation team.

Other rehabilitation options

A programme may also use ordinary treadmill walking, overground gait practice, an exercise bike, pool exercise, strengthening, balance drills, or walking aids. The choice depends on the operation or injury, weight-bearing instructions, movement quality, and access to equipment. See [Post-Operative Rehabilitation](/treatments/physiotherapy-and-rehabilitation/post-operative-rehabilitation/) for how supervised recovery may be organised.

What this means for you

The treadmill may let you rehearse a more natural walking pattern before your leg is ready for full loading. You should still follow restrictions about weight-bearing, braces, or movement. Tell the therapist if pain increases, your knee or ankle feels unstable, swelling changes after a session, or you feel dizzy or short of breath. Progress is based on your response and movement control, not on completing a particular speed.

Safety and supervision

Sessions should be set up and observed by a trained clinician. The harness, garment seal, emergency stop, and walking position need checking before movement begins. Stop and report chest discomfort, faintness, unusual breathlessness, sudden sharp pain, or a new loss of control. After surgery, the therapist must account for wound status, precautions, and the surgeon’s loading instructions before selecting the support level.

Questions people ask

Does an anti-gravity treadmill remove all weight from my legs?

No. The clinician selects partial support, so your legs still receive a controlled amount of loading unless a specific plan says otherwise.

Can I use it immediately after an operation?

Only when your surgical and rehabilitation team considers it appropriate. Wound status and weight-bearing restrictions must be checked first.
