What difficulty on stairs can feel like
Difficulty climbing stairs can reflect pain, weakness, or restricted movement in the hip, knee, or surrounding muscles.
You may feel pain as you bend the knee or hip, weakness when lifting the body, stiffness after sitting, or insecurity on a step. Some people manage stairs one at a time, use a rail, or find going down harder than going up. Note the painful joint, whether one side leads, and whether swelling, clicking, or giving way occurs.
When stair difficulty needs urgent care
Arrange urgent assessment after a fall with inability to bear weight, a visibly deformed joint, sudden major swelling, or a hot painful joint with fever. New leg weakness, loss of balance, or numbness that develops quickly also warrants prompt medical attention. Until assessed, use support and avoid testing the painful movement repeatedly.
Possible causes
These causes are alphabetical, not a likelihood ranking. Hip Osteoarthritis may limit hip movement and make steps painful. Knee Osteoarthritis can cause stiffness, pain, and reduced confidence when the knee bends or bears load. Patellofemoral Pain Syndrome may produce pain around the kneecap during bending, squatting, or stairs.
How clinicians investigate it
Assessment includes your stair pattern, walking, balance, hip and knee range of motion, tenderness, alignment, and muscle strength. The clinician may watch you sit, stand, squat, or step when safe. Your history helps distinguish pain-limited movement from true weakness. Imaging may be considered when examination findings suggest joint changes or another injury.
Treatment by cause
Physiotherapy may use strengthening for the thigh, hip, and trunk, alongside balance and step practice. Joint stiffness is addressed with movement and load planning. Footwear, handrail use, and pacing can make daily stairs safer. If severe joint damage continues to limit function despite non-surgical care, a specialist may discuss options such as Total Knee Replacement.
Preventing worsening stair difficulty
Keep joints moving within a comfortable range and build strength gradually rather than stopping all activity. Practise sit-to-stand control, maintain a clear stairway, and use a handrail when balance is uncertain. Break demanding tasks into manageable periods and address a new limp early so it does not become a persistent movement habit.
Other symptoms to track
Record pain at rest or with movement, swelling, stiffness duration, clicking, locking, giving way, and changes in walking distance. A short diary showing which direction on the stairs is harder can help the clinician identify the movement that needs testing.

Questions people ask
Why can going down stairs hurt more?
Going down requires controlled bending and load through the hip and knee, which can expose pain or weakness in those movements.
What should I do if my knee gives way on stairs?
Use the rail, avoid rushing, and arrange assessment, especially if giving way follows an injury or comes with swelling or locking.
