Dutasteride: what it is and how it is used for hair loss

Dutasteride, also known by the brand name Avodart, is a 5-alpha-reductase inhibitor. The enzyme normally helps convert testosterone into dihydrotestosterone, often called DHT. Dutasteride reduces this conversion, including through more than one form of the enzyme, so less DHT is available to act on sensitive hair follicles.

In hair restoration, a clinician may discuss dutasteride when a pattern of androgen-related thinning is suspected. The medicine can be taken by mouth or used in a procedure such as dutasteride mesotherapy, depending on the clinician's approach and local practice. These routes are not interchangeable, and the safety information differs.

By lowering DHT, dutasteride may reduce the hormonal signal that gradually miniaturises susceptible scalp follicles. The aim is usually to slow further thinning and support the conditions needed for existing follicles to produce stronger hairs. It cannot recreate a follicle that has been permanently lost, and a visible change requires time because hair grows in cycles.

Dutasteride can enter the bloodstream when taken orally, while a local injection procedure is intended to place medicine in selected scalp areas. Absorption and risk cannot be assumed to be absent with a local procedure. Possible concerns include sexual side effects, breast tenderness or enlargement, mood changes, and changes in semen-related measures; report these to the prescriber.

Dutasteride is licensed in some settings for prostate enlargement, while use for scalp hair loss may be outside the product licence depending on the country and route. In aesthetic and dermatology practice, it may be discussed for androgenetic alopecia after the clinician has examined the scalp and considered other causes such as alopecia areata, thyroid disease, iron deficiency, inflammation, or traction.

Dutasteride mesotherapy involves injections into the scalp by a trained professional. It is a treatment procedure, not a diagnosis, and it does not make a biopsy or blood tests unnecessary when the pattern of hair loss is unclear.

Depending on the diagnosis, alternatives may include topical minoxidil, oral minoxidil with medical supervision, finasteride, low-level light treatment, platelet-rich plasma, or a hair-transplant assessment. Some people need treatment for an underlying scalp disorder rather than a DHT-lowering medicine. Combining treatments can change the monitoring and side-effect profile, so the plan should be individualised.

A clinician can compare these choices by looking at the type and distribution of thinning, scalp findings, other medicines, blood pressure, pregnancy potential, and your priorities about maintenance. Ask which route is being offered and what evidence of benefit will be used at review.

Before treatment, provide a full medical history and mention liver disease, sexual or reproductive concerns, mood symptoms, breast changes, fertility plans, and all medicines or supplements. Dutasteride is not appropriate for people who are pregnant or may become pregnant because exposure can harm a developing male fetus. Women who are pregnant or could be pregnant should not handle leaking capsules.

A scalp consultation should establish the likely diagnosis and set realistic goals. Keep follow-up photographs in similar lighting, use the prescribed dose or procedure schedule, and report scalp infection, severe inflammation, new breast lumps, marked mood change, or troublesome sexual effects. Do not donate blood while taking dutasteride and for the interval advised by your clinician after stopping.

Hair benefits generally require ongoing treatment because the hormonal influence on susceptible follicles continues. If treatment is stopped, any benefit that depended on continued DHT reduction may fade over time. The prescriber should reassess benefit, side effects, and whether the diagnosis still supports treatment rather than extending it automatically.

For mesotherapy, safety also depends on sterile technique, injection depth, product handling, and management of scalp irritation. Tell the clinic about anticoagulants, allergies, active scalp infection, and previous reactions before a session. Seek medical care for spreading redness, increasing pain, pus, fever, or a sudden allergic reaction after treatment.
