What is a bone-patellar tendon-bone graft?

A bone-patellar tendon-bone graft uses a strip of patellar tendon with bone blocks at both ends to reconstruct a damaged ACL.

The graft is taken from the front of the knee, with one bone block from the kneecap and another from the shinbone. The tendon strip connects the blocks. During reconstruction, the surgeon positions this tissue along the ACL’s path and secures the bone blocks inside prepared tunnels.

The graft combines tendon tissue with bone ends that can sit within bone tunnels. Its shape allows the surgeon to prepare a defined replacement for the ligament. Where it is used

This graft is most closely associated with ACL reconstruction, an operation intended to restore stability after an anterior cruciate ligament injury. The surgeon removes or works around the damaged ligament, places the graft through bone tunnels, and fixes it so the tissue can heal into position. The operation is only one stage of recovery; progressive physiotherapy helps restore movement, strength, balance, and confidence.

Other graft choices

Other choices may include a hamstring tendon graft, a quadriceps tendon graft, or tissue from a donor. A choice is individual rather than a simple ranking. Previous knee pain, work and sport requirements, growth stage, prior surgery, and the condition of available tendons can all matter.

Because tissue is taken from the front of the knee, early recovery may include soreness when kneeling, using stairs, or contracting the quadriceps.
