Where and how the discomfort is felt
Joint pain is felt in or around a joint, while muscle aches are felt in the soft tissue that moves it.
A joint problem may be most noticeable with bending, straightening, gripping or bearing weight. A muscle ache may feel spread along a thigh, calf, shoulder or back, and can be tender when the muscle is pressed or stretched. Both can follow unusual activity, illness or an injury, so the exact location and movement that triggers pain are useful clues rather than a diagnosis.
Joint pain may fit better when
The discomfort seems to come from a knee, ankle, hip, wrist, elbow or shoulder itself rather than from the surrounding tissue. Stiffness after rest, a feeling that the joint catches, pain with a specific range of motion, or swelling focused around the joint can support that description. It is also helpful to note whether the joint feels unstable, locks, or has changed after a twist or fall.
Muscle aches may fit better when
The ache follows a workout, heavy lifting, a long journey, poor sleep, tension or a viral illness and is centred in the muscle rather than the joint. Soreness may appear across more than one muscle group and can make ordinary movements feel uncomfortable. A local muscle strain is more likely to hurt during a particular effort, such as climbing stairs or lifting an arm, especially if that muscle was overstretched or overloaded.
Symptoms that need prompt attention
Seek urgent medical assessment for a hot, red, very swollen joint, especially with fever or feeling generally unwell. Prompt care is also appropriate after a significant injury when you cannot use the limb, the joint looks out of place, or there is numbness, weakness or a cold pale hand or foot. Get urgent help for severe muscle pain with marked weakness, dark urine, or rapidly increasing swelling, particularly after crushing injury or extreme exertion.
When the source may be elsewhere
Pain around a limb is not always generated by a joint or muscle. Tendons, ligaments, nerves, skin and blood vessels can produce discomfort in the same area. Burning, tingling or pain travelling from the neck or back can point toward nerve irritation. Pain that is mainly in the bone, pain with a new rash, or swelling of one calf deserves a clinical assessment because the next steps depend on the pattern found on examination.
What a clinician considers
A clinician will usually ask when the pain started, what changed beforehand, which movements affect it and whether there is swelling, fever, weakness or other symptoms. Examination can compare both sides, check movement and strength, and identify whether tenderness is at the joint, muscle, tendon or another structure. Tests or imaging are selected when the history and examination suggest they would clarify the cause. Until you are assessed, avoid activity that sharply worsens the pain and keep note of changing symptoms.

Should I exercise through the pain?
