Isotretinoin is used for severe or persistent acne across several patterns, while spironolactone is mainly considered for hormonally influenced acne in suitable patients.

Isotretinoin directly reduces oil production and the tendency of pores to clog. Spironolactone blocks some androgen effects, which can reduce oiliness and breakouts when hormones contribute. Neither medicine is automatically right for every patient; acne distribution, scarring, pregnancy plans, medical conditions, and previous treatment response shape the recommendation.

Isotretinoin may be discussed when acne is deep, scarring, extensive, or persistent after consistent topical treatment and other appropriate options. It can make sense when the acne pattern is not confined to a hormonal distribution or when rapid control of ongoing inflammatory disease is a priority. The prescriber reviews mental health, liver health, blood fats, other medicines, and pregnancy potential. Skin dryness is common and usually requires a deliberate care routine.

Spironolactone may be considered when breakouts cluster around the jawline or lower face, fluctuate with the menstrual cycle, or occur alongside signs of androgen sensitivity such as unwanted facial hair. It is not suitable for everyone. The clinician checks blood pressure, kidney function where needed, medicines that raise potassium, and pregnancy plans. Changes in bleeding pattern or breast symptoms should be reported so the plan can be adjusted.

Isotretinoin can produce significant dryness, muscle or joint symptoms, blood-test changes, and mood changes that need prompt attention. It must not be used during pregnancy because it can seriously harm an unborn baby. Spironolactone can lower blood pressure, alter periods, cause breast tenderness, and raise potassium in people with certain health conditions or medicine combinations. If acne is mild, a topical retinoid, benzoyl peroxide, or another local treatment may offer a simpler starting point.
