When hydrotherapy may fit

Hydrotherapy may suit someone who finds land-based movement difficult because of pain, stiffness, weakness, or fear of loading. The water can allow a person to practise walking, balance, range of motion, or strengthening with support. It is especially useful when the pool environment helps someone begin moving while a therapist monitors technique and tolerance.

When physiotherapy may fit

Broader physiotherapy may be the better starting point when the main need is diagnosis, an individual exercise programme, gait or posture assessment, home advice, or training for a specific daily task. Land-based work can be progressed toward stairs, transfers, lifting, sport, or work demands. A physiotherapist can also decide whether pool sessions should be added later.

Safety and limitations

Pool sessions require attention to safe entry and exit, balance, supervision, water temperature, skin or wound status, and continence or infection concerns. People with certain heart, breathing, seizure, or medical conditions may need clearance before entering a therapeutic pool. Land-based physiotherapy can temporarily increase muscle soreness or joint symptoms when activity progresses. The therapist should adjust the load rather than pushing through sharp or escalating pain.

When urgent or different care is needed

Neither format should delay assessment after a major injury, sudden loss of strength, new numbness, severe swelling, fever, chest symptoms, or inability to use a limb safely. A rehabilitation programme may also need to wait while a fracture, infection, blood-flow problem, or serious neurological cause is assessed. The right next step could involve medical review, imaging, medication adjustment, or another specialist service.

How the rehabilitation choice is made

A clinician considers the diagnosis or working diagnosis, current mobility, pain response, balance, skin health, confidence in water, and the tasks the person wants to regain. They also review transport, pool access, supervision, and what can be practised between visits. A staged plan may begin in water for supported movement and transition to land-based strengthening, or use both formats when each teaches a different skill.
