A useful diary records sleep quality, meals and fluids, activity, stress, infections, medicine changes, and whether fatigue is constant or comes in waves.

A muscle cramp is the closer fit when

Use muscle cramp when a muscle suddenly tightens on its own and is painful or difficult to release. Cramps can occur during activity, at rest, or overnight. Describe which muscle was involved, what you were doing beforehand, how long the episode lasted, how often it returns, and whether you also have swelling, numbness, weakness, or back pain.

Warning signs that need faster care

Seek urgent care for fatigue with chest pain, severe shortness of breath, fainting, confusion, new weakness on one side, or trouble speaking. Seek prompt assessment for a painful swollen limb, a leg that becomes pale or cold, severe cramping with weakness, or cramps after a major injury. Do not drive yourself if sudden neurological symptoms or fainting are present.

Symptoms that should not be labelled as either

A twitch is a brief flickering movement and does not always cause the sustained tightness of a cramp. Muscle weakness is reduced power, such as difficulty lifting a foot or rising from a chair, and deserves separate attention. Tingling, burning, joint pain, or pain shooting from the back into a leg may point toward nerve, joint, or spine-related symptoms rather than fatigue or a simple cramp.

The consultation usually covers when symptoms began, exercise and work demands, sleep, fluid intake, diet, alcohol use, medical conditions, and medicines or supplements. Examination may check muscle tone, strength, reflexes, circulation, and tenderness.
