Fainting is a brief loss of consciousness, whereas palpitations are an awareness that the heartbeat feels unusual, fast, hard, or irregular.

Fainting, also called syncope, involves loss of posture followed by recovery. People may notice warmth, nausea, sweating, blurred vision, ringing in the ears, or light-headedness beforehand. Palpitations are the feeling of an unusual heartbeat; it may seem rapid, fluttering, pounding, skipped, or irregular. A person can have palpitations without fainting, and a heartbeat change can be noticed before a fainting episode.

When fainting is the better description

Choose fainting when you blacked out, lost awareness, or woke up on the floor or in a chair without remembering the period in between. If someone saw the event, ask what they noticed about your colour, movements, breathing, how long you were unresponsive, and how you recovered. Near-fainting is also worth describing: you may have felt that you would pass out but managed to sit or lie down before losing consciousness.

When palpitations is the better description

Choose palpitations when you remained conscious but could feel your heart beating differently. Note whether the sensation began suddenly or gradually, whether it was steady or irregular, and whether it occurred at rest, during exertion, after a stimulant, or during emotional stress. Breathlessness, chest discomfort, dizziness, fever, dehydration, and new medicines can all be relevant details for the assessment.

When to seek urgent help

Seek emergency care now for fainting during exertion, fainting with chest pain, severe shortness of breath, a persistent racing or irregular heartbeat with feeling unwell, or new weakness, trouble speaking, or severe headache. Emergency assessment is also needed after a serious injury from a collapse. These symptoms require rapid review because they may signal a heart rhythm problem, reduced blood flow, a lung problem, bleeding, or a neurological emergency.

Other experiences that can be confused with either

Dizziness is not always fainting, and a feeling of panic is not always palpitations, although either may occur with them. Vertigo creates a spinning sensation without loss of consciousness. Low energy, breathlessness, tremor, chest discomfort, or a sensation of missed beats can have several possible causes. Avoid driving, climbing, swimming alone, or operating machinery until recurrent blackouts or significant dizziness have been medically assessed.

How clinicians investigate

A clinician will ask about the event, medical history, family history of sudden cardiac problems, medicines, alcohol or stimulant use, and whether a witness was present. They may examine the heart and nervous system, check blood pressure lying and standing, and record an ECG. Blood tests or heart rhythm monitoring may be considered when the pattern suggests they could help. Accurate notes about timing and activity can make intermittent symptoms easier to evaluate.
