Chest X-Ray vs Spirometry

A chest X-ray shows chest structures, while spirometry measures how much and how quickly you breathe out.

These tests answer different clinical questions. A chest X-ray creates an image of the lungs, heart outline, ribs, and other chest structures. Spirometry is a breathing test that records airflow while you breathe into a mouthpiece. An X-ray may help identify a structural change; spirometry may show a pattern of airflow limitation. One test does not replace the other, and a clinician may request both when symptoms and examination findings point to more than one possibility.

A clinician may choose a chest X-ray for symptoms or examination findings that raise a question about the lungs, chest wall, or heart size and shape. Cough, fever, chest injury, or unexplained chest symptoms may lead to imaging depending on the overall assessment. The image is quick to obtain, but it cannot directly measure airflow. A normal image can be reassuring in some situations while still leaving a breathing-function question unanswered.

Spirometry is useful when the concern is how air moves through the airways, such as recurring wheeze, cough, breathlessness, or a suspected obstructive breathing pattern. Results depend on a strong, consistent effort, so the staff member will coach you through the manoeuvre and may repeat it. Recent illness, chest discomfort, dizziness, or difficulty performing the breaths can affect whether testing is suitable that day.

A chest X-ray uses ionising radiation, so tell the imaging team if you could be pregnant or have questions about the examination. Spirometry does not use radiation, but forceful breathing can briefly cause coughing, light-headedness, or chest discomfort. Tell the team about recent surgery, a serious respiratory infection, or symptoms that make repeated deep breaths difficult. Staff can pause or stop either test if you feel unwell.

Neither test is the right first response to emergency warning signs. Severe breathlessness, fainting, blue or grey lips, confusion, or chest pressure with sweating or nausea needs urgent assessment. In other situations, the clinician may start with a physical examination, oxygen measurement, ECG, blood tests, or a different scan. The best test is the one that addresses the current question, not simply the test that is easiest to arrange.

The decision starts with your symptoms, their pattern over time, medical history, medicines, smoking or vaping exposure, and examination. Imaging is more informative when a structural cause is being considered. Breathing measurements are more informative when airflow is the focus. Clinicians also consider whether you can safely complete the test and whether a result would change the next decision. Ask what question the test is intended to answer and how the result will guide follow-up.
