Chemotherapy vs HER2-Targeted Therapy

Chemotherapy affects cells that divide quickly throughout the body. HER2-targeted therapy is designed for cancers whose cells have excess HER2, a growth-related protein. The choice depends on the tumour's HER2 testing, cancer type and stage, treatment aim, previous therapy, and overall health.

Chemotherapy affects dividing cells broadly, while HER2-targeted therapy focuses on cancer cells with excess HER2; some treatment plans use both.

Chemotherapy may be considered when cancer cells need a broad systemic treatment, when the tumour is not shown to be HER2-positive, or when chemotherapy is part of a plan before or after surgery. It can also be selected because the cancer has returned or spread and the care team needs an approach that reaches cancer cells in multiple parts of the body.

HER2-targeted therapy may be considered when testing shows that the cancer has excess HER2 and the medicine matches the cancer type and treatment setting. It may be given before surgery, after surgery, or for advanced disease. A HER2 result does not select one medicine by itself; the clinician also considers prior treatment and heart health.

Chemotherapy can cause tiredness, nausea, appetite changes, mouth soreness, hair loss, and changes in blood counts; the exact effects depend on the medicines used. HER2-targeted therapy can cause infusion reactions, diarrhoea, tiredness, or effects on heart function with some medicines. Monitoring may include symptom checks, blood tests, and heart assessments when appropriate. Report fever, breathing difficulty, chest symptoms, severe diarrhoea, or a sudden infusion reaction promptly.

Some cancers need surgery, radiotherapy, hormone therapy, immunotherapy, another targeted medicine, or supportive treatment instead. A tumour may have HER2 features but still require a different combination because of its location, stage, or previous treatment. If testing is incomplete or results are unclear, the team may arrange additional pathology review before choosing a HER2-directed medicine.

The care team starts with the pathology report, including cancer type and HER2 status, then considers stage, treatment goal, whether surgery is planned, and medicines already received. Kidney, liver, nerve, blood-count, and heart considerations can influence the plan. The team weighs expected benefit against likely side effects, explains whether treatment is given by infusion or another route, and sets monitoring points. Ask which test supports the recommendation, what the first treatment goal is, and which symptoms need same-day advice.
