Blocked nose vs headache: how the symptoms differ

A blocked nose is reduced airflow through one or both nostrils, while a headache is pain or pressure felt in the head or face.

A cold, allergy, sinus irritation, or dry air can cause nasal blockage and may also be followed by head or facial pressure. However, a headache can occur without a nose problem, and nasal congestion does not explain every headache. Identifying the main symptom helps you choose sensible self-care and explain the problem clearly if you need an appointment.

Choose blocked nose when the main difficulty is getting air through your nose, especially when lying down or during a cold. You may breathe through your mouth, snore, notice a reduced sense of smell, or feel that one nostril is more closed than the other. Saline rinses or sprays can be helpful for some people, but follow product instructions and seek advice before using medicated decongestants if you have relevant health conditions or take regular medicines.

Choose headache when pain or pressure is the main concern, even if you also have a stuffy nose. Describe where it is felt, whether it came on suddenly or gradually, and whether activity, bending, light, sound, coughing, or sleep changes it. Rest, regular fluids, and avoiding missed meals may help some common headaches, but repeated or changing headaches deserve clinical review.

Get urgent medical help for a sudden, extremely severe headache; headache after a head injury; headache with confusion, fainting, seizure, weakness, trouble speaking, vision loss, a stiff neck, high fever, or a new rash. Urgent assessment is also appropriate if nasal swelling spreads around an eye, there is severe eye pain, or vision changes. These signs are driven by their seriousness, not by whether congestion is present.

A runny nose is discharge from the nostrils rather than blockage, although both can occur together. Facial tooth pain or ear pressure may point to a different area of discomfort. If you have ongoing mouth breathing, snoring, recurrent nosebleeds, a persistent one-sided blockage, or a headache pattern that is new for you, arrange a medical assessment rather than simply changing over-the-counter products.

A clinician may ask about allergies, recent viral illness, injury, sleep, hydration, caffeine, medicines, and the timing of symptoms. They may look inside the nose, check the eyes, ears, throat, blood pressure, and neurological function, and examine the face and neck. This helps decide whether the symptoms fit a minor illness, allergy, a primary headache disorder, sinus involvement, medication effects, or a problem needing further tests.
