What should you know about zolmitriptan for cluster headache?

Cluster headache is typically marked by attacks of intense pain around one eye or one side of the head, often with a watery eye, blocked or runny nose, eyelid changes or restlessness on the painful side. Zolmitriptan belongs to the triptan group. It is intended to stop or lessen an attack after it starts; it is not usually a medicine for taking every day to prevent attacks.

The prescribed form and timing matter. Follow the instructions on your prescription and do not take extra doses, combine it with another triptan, or use it with an ergot-containing headache medicine unless the prescriber has given clear instructions.

Before starting zolmitriptan, make sure the prescriber knows about heart or blood-vessel disease, previous stroke or transient neurological symptoms, uncontrolled high blood pressure, liver disease, pregnancy, and all medicines or supplements you use. Some antidepressants and other medicines can require closer review because of interactions.

Record when attacks start, which side is affected, associated symptoms, medication taken and the response. This diary can help a clinician confirm the headache pattern and decide whether you also need preventive treatment during a cluster period.

Seek emergency help for a sudden severe headache that reaches maximum intensity quickly, a headache after a significant head injury, fainting, seizure, confusion, weakness, trouble speaking, new vision loss, fever with a stiff neck, or a new severe headache during pregnancy. Seek urgent review for chest pain, shortness of breath, marked tightness or weakness after taking zolmitriptan. A new or changing headache pattern also needs medical assessment.
