Zinc for nappy rash: what to know

Nappy rash often develops where dampness, contact with urine or stool, rubbing, and occlusion irritate the skin. A zinc barrier forms a protective layer over the affected area, helping reduce direct contact with moisture while the skin settles. It is a topical product, not the same as taking zinc by mouth.

A rash that is bright red, affects skin folds, has small spots around its edge, blisters, crusting, or keeps returning may have another contributor, such as yeast or infection. The appearance alone cannot confirm the cause, and scrubbing can increase irritation.

Change nappies frequently, clean gently with water or a fragrance-free cleanser, and pat rather than rub. Allow the skin to dry, then apply a thin, even layer of zinc barrier product according to its label. Avoid fragranced wipes, harsh soaps, and vigorous removal of every trace if the skin is sore; gently loosen residue during the next clean.

If the rash does not settle with careful barrier care, a clinician may recommend Emollient Therapy or, when yeast is suspected, Topical Antifungal Treatment. Do not use a steroid, antifungal, or antibiotic product on an infant without appropriate advice.

Seek medical advice if the rash is spreading, very painful, bleeding, blistered, oozing, associated with fever or unusual sleepiness, or not improving with gentle care. A baby who seems unwell, feeds poorly, or has fewer wet nappies needs prompt assessment. Get advice sooner for a young infant or a child with recurrent rash.

Ask what is most likely irritating the skin, how often to apply the barrier, and whether the folds or rash pattern suggest yeast. Check which cleansers and products are suitable for your child and when the rash should be reviewed.
